Join the Michigan HomeCare & Hospice Association for a complimentary Lunch & Learn webinar as we explore the Guiding an Improved Dementia Experience (GUIDE) Model recently launched by the Centers for Medicare & Medicaid Services (CMS). 

This session, presented in partnership with Nevvon and PocketRN, will give members an overview of how the GUIDE program works, who is eligible to participate, and the potential impact on home health, hospice, and personal care providers. Attendees will gain practical insights into how this initiative aims to support individuals living with dementia and their caregivers through enhanced coordination, services, and resources.
