Description
The 2025 hospice final rule included the anticipated Hospice Outcomes and Patient Evaluation (HOPE) tool. This session will outline the implementation date for the HOPE tool and how it differs from the Hospice Item Set. It will review the current tool and discuss the completion requirements. You’ll also learn the impact on the Hospice Quality Reporting Program as new measures will be added resulting from clinical data collection on the HOPE.
