The much-anticipated HOPE tool is a standardized assessment that will help providers gain a better understanding of patients’ needs, which can help build a more individualized care plan.
Understand the requirements to accurately complete the HOPE tool assessment
Apply guidance to accurately complete these sections:
Section A – Administrative Information
Section F – Preferences for Customary Routine and Activities
Section I – Active Diagnoses
This session will provide clear guidance for completion of the following sections of the HOPE tool: A-Administrative, F-Preferences, and I–Active Diagnoses. It will address the importance of accurately capturing patient-specific data, including patient preferences related to CPR and other life-sustaining treatments, hospitalization, and spiritual/existential concerns. It will also cover the principal diagnosis, comorbidities, and coexisting conditions to guide and positively impact service delivery. Item-specific instruction will be provided along with tips to ensure accuracy.
