Home Health serves a high-risk population   elderly patients and those with complex medical and social comorbidities   addressing factors that contribute to rehospitalization is critical to improving your scores. This webinar will emphasize the importance of focusing on patient mental health as a strategy to reduce rehospitalizations\, a key metric that is both monitored and incentivized through HHVBP and other similar programs. By integrating mental health management into patient care\, providers can not only improve overall patient outcomes but also positively impact their quality performance.
