Understanding the GUIDE Model — and Why It Matters for Home Care & Hospice

Supporting individuals living with dementia, and the families who care for them, is one of the most pressing needs in today’s health care landscape. The Centers for Medicare & Medicaid Services (CMS) is piloting an innovative new approach called the GUIDE Model (Guiding an Improved Dementia Experience). This voluntary, value-based care model is designed to improve the quality of life for people with dementia and their caregivers while helping agencies deliver more coordinated, person-centered services.

Under GUIDE, participating organizations will receive new payments for providing comprehensive dementia care, including:

A care navigator to guide families through available resources and services.

24/7 access to support for urgent needs and questions.

Caregiver education and respite services to reduce burnout and keep loved ones safely at home longer.

Enhanced coordination across primary care, specialists, and home-based providers.

For home health, hospice, and private duty providers, GUIDE represents both a clinical opportunity and a business decision. Agencies that understand how the model works can explore partnerships, assess their readiness to participate, and plan for workforce and operational changes that support dementia-capable care.
