This session will provide clear guidance for completion of the following sections: J-Health Conditions, addressing the imminence of death, pain, dyspnea, symptom impact screenings, and follow up; M-Skin Conditions, including wounds, ulcers, and treatment; N-Medications, including opioid administration and bowel regimen; and Z-Record Administration, covering requirements of all agency-determined staff who may contribute to the completion of the HOPE tool and verification of completion and signatures.
